In IVF, the fertilization date is already known, so the classic Naegele Rule, which estimates ovulation from the LMP, becomes unnecessary. What changes the calculation is a single extra piece of data: the embryo's stage on the day of transfer, day 3 or day-5 blastocyst, because that number sets how many days to add to the transfer date.
The sum is 266 minus the 5 days the embryo had already developed by the time of transfer, resulting in 261 days added to the entered date.
With only 3 days of development subtracted from 266, 263 days remain, two more than in the blastocyst protocol, a result 2 days later than the same transfer date.
Entering that adjusted LMP in the standard EDD calculator, the one that only accepts an LMP, produces exactly the same result as the direct days-after-transfer calculation, because both are the same sum.
Naegele's Rule adds 280 days (40 weeks) to the last menstrual period date to estimate the due date. It assumes a 28-day cycle and ovulation on day 14. Longer or shorter cycles shift the actual time of ovulation, which is why ultrasound is more accurate for confirming or revising the individual EDD.
Because Naegele's Rule exists to estimate an unknown fertilization date, assuming ovulation on day 14 of a 28-day cycle. In IVF the transfer date, and the embryo's exact stage, are already known precisely, so the estimate becomes a deterministic sum of days instead of a guess about the menstrual cycle.
It is a fixed 2-day difference in the final EDD: 261 days added to the transfer for a blastocyst, 263 for day 3. The blastocyst has already spent 2 more days developing by the time of transfer, so it needs 2 fewer days added afterward to reach the same 266-day total since fertilization.
Not directly: the calculator's only field is labeled LMP. You need to work out the adjusted LMP first, subtracting 19 days from the transfer date for a blastocyst or 17 days for a day-3 embryo, and enter that adjusted date in the LMP field to get the correct EDD.
